2. Documentary evidence on disability(ies) - Please refer to Part V of the Guidance Notes on Application for the “Registration Card for People with Disabilities” (“Guidance Notes”) on the following page regarding the requirement on the documentary evidence on disability(ies). Some examples are as follows: certificates issued by doctors or allied health personnel registered in Hong Kong, or officer-in-charge of relevant organizations; “Certification of Disability Type for Registration Card for People with Disabilities” issued by doctors or allied health personnel registered in Hong Kong, or officer-in-charge of relevant organizations (“CRR4”) (Click here to download); or“Consent Form – authorization for data checking from Social Welfare Department” (“CRR/SWD1”) (Click here to download) to authorise the CRR to verify the applicants’ disability-related information with the Social Welfare Department

3. a recent colour photograph (a full-faced colour photograph taken within the last six months with a plain background)

3. A colour photograph (a colour passport photo within the last six months with a plain background) [only applicable to Child/juvenile upon attaining the age of 11 and 18 respectively]

4. A letter stating the reasons for early renewal application [only applicable to cardholder submit his/her application for renewal more than two months before the expiry date as shown on his/her Card]

(i) For lost cards
